A Seamless Path from Youth to Adulthood

Why Both?

FMA Universal aims to bridge the gap between young people and adults by offering continuity for existing young persons who are currently in our care to transition to adulthood in cases requiring ongoing support. By Offering (18- 65), we ensure a smooth, uninterrupted transition, reducing the risk of homelessness, disengagement, or deterioration in both mental and physical health. This will also allow FMA Universal to also address other unmet needs of (18-65) with other disabilities, mental health challenges or complex social needs often facing long waiting lists for supported living. Combining both services allows us to help address this gap efficiently.

Supported Transitioning and Community Reintegration

We offer single point of access, care and support for both existing and newly referred individuals. Our young person’s already in our care (16-18) into adult care ( 18-65) are not yet able to live independent therefore require ongoing support. This continuation ensures smooth , uninterpreted transition thereby reducing risk of homelessness, disengagement or further deterioration in their physical and mental health. Taking adults clients coming from inpatient services allows us to address some of the pressure of unmet needs of (18-65) who are struggling to be integrated back into the community due to lack of adequate support to meet these needs. Thus, our experienced support system will help create personalised care, build their independence whilst fostering and building engagements with their communities, professionals involved and their families.
